Please print and completely fill out form 1 and complete form 2 only if necessary. You will be bringing in 7 completed forms to your first appointment. Read directions for needed info on the body diagram form (under form 4). Call us if you have any questions.

Please indicate areas of current discomfort, any scars and pain. If having pain, please rank on a scale of 1-10 (1 being no pain to 10 being the worst pain ever)

All minors under the age of 18 must have a parent present with them during their massage and have the parental consent form signed before their massage can take place. Please print and sign and bring with you to your appointment. Please refer to the Under 18 Policy'
